Welcome to the Healthier Minnesota Community Clinic Fund

The Healthier Minnesota Community Clinic Fund (Foundation) is an independent foundation created to improve the quality of care for low income people, communities of color, and the medically underserved. The Foundation will accomplish its mission by providing grants to community clinics.

The Foundation funds primary care safety net providers through its Traditional Community Clinic grants.

Traditional Community Clinic Grants: The 2008 Traditional Community Clinic RFP is located under the 2008 Traditional Community Clinic grant tab.

Innovations Grants: This grant category is no longer available.

In 2007, the Healthier Minnesota Community Clinic Fund awarded more than $4.9 million in grants in the Traditional Community Clinic category.

In 2006, the Healthier Minnesota Community Clinic Fund awarded more than $1.7 million in grants in the Traditional Community Clinic ategory.

In 2005, the Healthier Minnesota Community Clinic Fund awarded over $1.3 million in one-year grants to safety net providers across Minnesota.

In 2004, the Healthier Minnesota Community Clinic Fund awarded more than $1.8 million to safety net providers across Minnesota.
